Horizon Health Network in beautiful New Brunswick is looking for a Primary Health Care Manager at the Fredericton Downtown Community Health Center Manager to join our team!
Primary Health Care Manager
Works with staff and communities served by Primary Health Care Centres in the Fredericton and Upper River Valley area to facilitate access to comprehensive Primary Health Care Services; responsible for ensuring that programs and services are aligned with the needs of communities served; and the Strategic directions of Province and Regional Health Authority.
Currently this management position will be responsible for the Fredericton Downtown Community Health Centre, with the possibility of more sites in the area in the future.
This site has a unique shared leadership model with our partner - the University of New Brunswick Faculty of Nursing (UNB). The manager works jointly with the UNB Nurse Manager in the daily operations of the FDCHC. They share joint accountability for upholding the vision and mission of the FDCHC to ensure that service delivery, clinical practice, teaching, learning and research are supported.

Qualifications:
- Bachelor's degree in a healthcare discipline or related field;
- Must have a minimum of 5 years of leadership experience;
- Demonstrated skills in facilitating, team building, and conflict resolution;
- Demonstrated ability in planning, budgeting, information analysis and reporting;
- Experience negotiating services and resources between programs and partners;
- Demonstrated knowledge of community partners and resources;
- Demonstrates excellent communication and interpersonal skills;
- Demonstrated experience in handling issues and information of a confidential and sensitive nature.
- Ability to attend work on a regular basis.
- Ability to perform the duties of the position.
- Good work record.
- Written and spoken competence in English is required
What you will do
In collaboration with the UNB Nurse Manager:
- Plans and evaluates overall operations and programming for the FDCHC;
- Overall responsibility for budget and allocation of resources to respond to the health centres' needs/population served;
- Works to ensure appropriate staffing levels;
- Is responsible for issues related to staff; e.g. Attendance Management and/or progressive disciplinary processes.
- Ensures clinical practice guidelines, nursing standards, and community standards are met.
- Collaborates with providers and staff at community health centres to ensure access to appropriate provider is timely (based on Third Next Available reports, roster size and patient visits/provider).
- Facilitates collaborative approaches within interdisciplinary team as to provide comprehensive care.
- Supports mental health, population health and chronic disease prevention management practices within the teams.
- Identifies new service requirements and deficiencies in current services; and will work with Primary Health Care Access Coordinator, Community Advisory Committees, and others to develop implementation plans.
- Participates in recruitment including job postings, interviews, and orientation.
- Accepts requests for education and approves based on Horizon process and policy.
- Reports activities of all health centres monthly to Director in regard to current status and future plans.
- Performance Appraisals of all reporting staff
- Works with Medical Liaison(s) and Clinical Department Head of Family Medicine to ensure open communication between medical staff and Horizon.
- Maintains good knowledge of community resources in order to better meet client needs.
- Will work with Primary Health Care Access Coordinator and other programs such as Mental Health and Addictions to ensure integration of resources and programming to respond to identified needs of community.
- Attends Team meetings and leads appropriate staff meetings for direction and support.
- Serves on Community Health Centres Needs Assessment work groups, as appropriate
