Role Description The Service Coordinator is a full-time, on-site role based in Guelph, ON, responsible for organizing and overseeing service delivery for clients and programs. Day-to-day tasks include coordinating client intake and referrals, scheduling services, and ensuring that service plans are implemented efficiently and consistently. The role involves regular communication with clients, families, internal teams, and external partners to clarify needs, provide updates, and resolve issues. The Service Coordinator monitors service quality, supports operational workflows, and maintains accurate documentation in alignment with organizational standards and policies. This position contributes directly to a supportive care experience by helping clients access timely, appropriate mental health and addiction services. Qualifications Candidates should possess strong Service Coordination skills, including organizing client services, managing referrals, and supporting care pathways. Candidates should possess excellent Interpersonal Skills to build rapport, work collaboratively with multidisciplinary teams, and foster respectful, inclusive interactions. Candidates should possess clear and professional Communication skills, both written and verbal, to convey information accurately and maintain thorough records. Candidates should possess Operations Management capabilities and Customer Service skills to support efficient workflows, address inquiries, and enhance client experience. Relevant post-secondary education in health services, social work, psychology, administration, or a related field is an asset. Experience in mental health, addictions, or healthcare coordination, and familiarity with confidentiality and privacy standards, is strongly preferred. Proficiency with scheduling systems and basic office software, strong organizational skills, and the ability to prioritize in a fast-paced environment are beneficial. Demonstrated commitment to equity, diversity, and inclusion, and the ability to work with clients from diverse backgrounds, is expected.
